Privacy of my clients personal information is important. Personal information will be collected, used, and disclosed in a responsible manner.

PERSONAL INFORMATION

The collection of personal information will be for the purpose of providing professional oral health care, and for providing information necessary to administer CARING FOR SMILES.

Personal information is the information provided on the medical/dental health history.

INFORMATION PROTECTION

It is my commitment to protect all your personal information. Security measures are in place to manage and safeguard your personal information from unauthorized access and theft. Your information will have limited access and will only be provided on a “need to know” basis.

INFORMATION DISCLOSUIRE

Specific information will be only be provided to those who have a need to know. Those who have a need to know include dentist and/or health care providers. It is my personal commitment to your privacy, that I will inform you of the intention to provide this information. The personnel information may also be provided to your dental insurance provider but I will inform you of my intent to do so. You may at any time designate any restrictions as to whom your personal information is disclosed and/or restricted to.

INFORMATION RETENTION AND DESTRUCTION

Personal information will be retained during the period that oral health services are being provided and during the necessary administration. Information is always destroyed in a secure manner if and when the information is no longer necessary for the provision of health care services and is not required to be retained for the compliance of provincial or federal regulations or statues.

YOUR PERSONAL ACCESS TO YOUR RECORDS

You will be provided with an open access policy to your personal information held by CARING FOR SMILES. At any time your records are available to be seen and you may request amendments be made to the personal information. Access will be provided within a reasonable amount of time causing CARING FOR SMILES to have minimal interruption.
